Peter E. Gutierrez, M.D. of Crown Point, IN, March 30, 1928 – August 31, 2012

 

Peter E.Gutierrez, M.D. age 84, of Crown Point, passed away Friday, August 31, 2012. He was born in Gary on March 30, 1928. He dedicated his life to health care and the Northwest Indiana Community. Peter attended Horace Mann High School in Gary. He attended the Citadel, Military College of South Carolina, Loyola U., Chicago, B. S. Biological Science; Loyola University, Chicago Bacteriology, Illinois Institute of Technology, Bacteriology, Indiana University School of Medicine, M.D. 1958, Internship, Methodist Hospital of Gary, 1958-1959. He served in the U. S. Army 1952 to 1954. He practiced in Crown Point, Indiana, 1959 to date and founded Crown Point Clinic, Inc., a multi-specialty group until 1968. Medical Director of James O. Parramore Hospital; Peter served as Medical Director Lake County Jail 1969 to 1982; Director of Professional Services, Lutheran  Home of Northwest Indiana 1980 to 1983; Past Chairman Peer Review Committee; Lake County Medical Society; Chairman Blue Shield State Wide Fee Review Committee 193 to 1975. He was Medical Director and Consultant on Utilization Review for Health Care Alternatives; Chairman Utilization Management Committee for St. Anthony Nursing Home; Chairman Utilization Management Committee for St. Anthony Hospital; Chairman Utilization Management Committee for St. Anthony Rehabilitation Unit; President Lake County Medical Center Development; Consultant to Indiana Medical Review Organization; Consultant to Office of Hearing and Appeals, Social Security Administration; Chairman Sanction Committee for Sentinel Medical Review Organization; Faculty member, Indiana University School of Medicine at Indiana University Northwest 1990 to date; Associate Medical Director, HealthChicago HMO March 1993 to January 1996; Chairman HUMAN, IPA HMO Pharmacy and Therapeutics Committee; Consultant for the Methodist Hospitals in Risk Management; Lake County Sheriff’s Medical Consultant currently serving as the Sheriff’s Department’s Chief Medical Officer since September 1, 2006; Chairman Continuous Quality Improvement Committee Lake County Jail; Medical Consultant to the Health Services Division of Lake County Jail; Guest lecturer on Medical Care in Correctional Institutions; Consultant to a local surgeon on Risk Management; Certified by the Social Security Administration to serve as a Medical Expert in Adult and Child Cases. He was a member of: American College of Physician Executives; American Board of Quality Assurance and Utilization Review Physicians (ABQAURP); American College of Medical Quality Vice-President Great Lakes District (1989 through 1990); American Medical Association; American Medical Peer Review Association; Indiana Association of Quality Assurance Professionals; Indiana State Medical Association; Lake County Medical Society; Lake County Medical Center Development Agency Board of Directors; National Association for Healthcare Quality and numerous social and community organizations .

 

He was preceded in death by his parents, Peter L. and Lucille Gutierrez and sister, Vinnie. He is survived by his wife of 26 years, Jacqueline Gutierrez; 4 children: Marie Vogel of Phoenix, Peter M. (Kim) Gutierrez of Phoenix, Jean (Mike) Powers of Kildeer, IL and Lucy (Anthony) Trotman of Camden, NC; stepchildren: Robyn (Tim) (nee Henderson) McGrath of Alexandria, VA, Amy (Joe) (nee Henderson) LeDonne of Austin, TX, Ross (Kristin) Henderson of Indianapolis, Lori and John Laszlo of Munster, Brian Fuchs of Crown Point and Sam Fuchs of Atlanta, GA. Numerous grandchildren and great grandchildren and many nieces and nephews.

 

Friends may visit with the family at the Burns Funeral Home, 10101 Broadway, Crown Point, on Tuesday, September 4, 2012 from 2:00 to 8:00 p.m. Funeral services will be held on Wednesday, September 5, 2012 at 10:00 a.m. at Burns Funeral Home with Rev. John Starr officiating. Services will terminate at the funeral home with cremation to follow at the Burns Funeral Home Crematory.
